Terms and Conditions

1. Service Agreement

David Lee, the personal naturotherapist/trainer, subcontractors of Vital Health (i.e. trainers, nutritionists, etc.) and the client (identified above) acknowledge an agreement wherein the client receives the aforementioned number of consultations and training sessions with David Lee and/or any other Vital Health Team member in return for the stated amount. All parties acknowledge that content, duration, and frequency of such sessions are defined by the Terms and Conditions of this agreement, which the client has read and understood.

2. Refund Policy - Health Assessments and Therapy/Nutrition Packages

The client agrees and understands that for the sessions of health assessments package and sessions of therapy/nutrition package including the report, no refund is available.

3. Refund Policy - Online Courses

All online courses purchased are non-refundable. Once an online course has been purchased and access has been granted, no refunds will be provided under any circumstances.

4. Refund Policy - QRMA Device

Payment for QRMA devices is only refundable if our technician checks the device remotely and confirms that the device is faulty. Refunds will not be provided for any other reasons, including but not limited to change of mind, dissatisfaction with the product, or user error.

For packages that include a QRMA device with an online course: If the device is found to be faulty by our technician through remote verification, you will be eligible for a refund of $139 only (representing the device portion of the package), and not the full package cost. The online course portion of the package remains non-refundable as per Section 3.

5. Refund Policy - Fitness and Nutrition Packages

For fitness and nutrition packages of 10 sessions or more that have been bought, there will be a refund minus sessions completed and administrative charges of $100 up to 10 days after the signing of this agreement. No refund will be provided after 10 days.

6. Cancellation Policy

If a twenty-four (24) hour notice of cancellation of a session is not given to David Lee or the trainer/nutritionist or other Vital Health employee, the client will be responsible for the full fee of the session. The exception is when the client has no control of the situation.

7. Session Freeze Policy

If the client wishes to freeze their session times to a later period up to a period of one month for the services of the naturotherapist/trainer David Lee and/or any subcontractors of Vital Health, that is optional for the client. However, if the freeze time period goes beyond one month, the client loses their number of sessions agreed upon in the contract and the contract becomes void, unless the client is seriously ill, has had surgery, or at the discretion of David Lee or the nutritionist/trainer/subcontractor of Vital Health.

8. Release of Liability and Waiver

In consideration of the acceptance of the registration application, I hereby for myself, my heirs, executors, administrators, and assigns, waive, release and discharge any and all rights I may have against naturotherapist/trainer: David Lee, his partners, employees, sponsors, officers, members and facility owners, employers and further release and save harmless naturotherapist/trainer David Lee, as well as all Vital Health employees from any damages which I may suffer in the fitness, therapy and nutrition program.

9. Physical Fitness Declaration

I declare and warrant that I am physically fit and that I am entering this program with the express knowledge and approval of my physician. I further acknowledge that I have to follow all instructions applicable to conducting my activities in a safe and prudent manner.

By using our services, you acknowledge that you have read, understood, and agree to be bound by these Terms and Conditions.
